Concentrated refuse and human or animal feces require removal. Rules, drains and designated disposal areas reshape streets and labor, while poorer groups may remain closest to hazards.

Skeletal evidence from Harappa and other sites shows lesions and trauma distributed unevenly. Disease is biological, but vulnerability follows nutrition, work, status, crowding and access to care.

People could observe that certain places, seasons or contacts were dangerous without knowing microbes. Urban design reflects many goals—defense, ritual, trade and power—so disease is one influence, not the master plan.
